If you are a member of an insurance company, and you chose us to be your dental healthcare provider as a participating dentist for that insurance company, it is your responsibility to provide us with the information pertaining to the claim, including your insurance card, member id number and personal information. You will find the information on patient registration form.

You will be responsible for your deductible and co-payment at the time of service. You are fully responsible for the services not covered by your insurance.
